Terms of Service

FormProof is provided by OMASSENO LLC, a Texas limited liability company ("FormProof", "we"). Contact: support@getformproof.com.

1. The service

FormProof checks one HubSpot form embedded with HubSpot's standard embed code on one page of a website. Each check confirms the embed code is in the page's HTML, confirms our HubSpot access, reads the form's settings, sends one labeled test submission through HubSpot's form submission service, searches your HubSpot CRM for the resulting contact for up to 15 minutes, and deletes that test contact. It does not test your website's Submit button, display in visitors' browsers, notifications or workflows, non-HubSpot forms, or lead quality. Each result describes one test at one time and is not a guarantee that any inquiry will be delivered.

2. Beta

The service is an invite-only beta, free of charge and provided "as is". Features, limits and availability may change. Paid plans start only if you opt in after we email you the price; nothing is charged automatically.

3. Your authorization

Before any test is sent you sign the authorization, confirming you are authorized by the business that owns the website and its HubSpot account. You register only websites and HubSpot accounts you control or are authorized to manage, and you tell recipients of HubSpot notifications that test inquiries need no action.

4. Limits

One page and one form per account; 4 scheduled and up to 4 on-demand checks per 30 days. Beta places are limited.

5. HubSpot access

You grant access on HubSpot's consent screen: read contacts and forms, and create or delete contacts. FormProof deletes only the test contacts it created. You can disconnect in Settings or by uninstalling the app in HubSpot.

6. Records left in HubSpot

Deleted test contacts stay restorable in HubSpot for 90 days; form submission history entries remain; spam-filtered tests are removed by HubSpot after 90 days; notifications or workflows a test triggers are not undone.

7. Acceptable use

Don't use FormProof to test sites or accounts you aren't authorized for, to send anything other than its own labeled tests, or to overload HubSpot or any website. We may suspend accounts that do.

8. Support

Support is by email only. No calls are required for setup or use.

9. Disclaimer and liability

The beta is provided without warranties of any kind. To the extent permitted by law, our total liability for the free beta is limited to USD 0, and we are not liable for lost leads, revenue or indirect damages.

10. Termination

You can delete your account in Settings at any time. We may end the beta with 14 days' email notice.

11. Governing law

These terms are governed by the laws of the State of Texas, United States, without regard to its conflict-of-law rules.

12. Changes

We will email account holders before material changes take effect.
